Erectile dysfunction, which is popularly called impotence, can occur due to many factors.

Erectile dysfunction, which can be defined as a man's recurrent inability to achieve sufficient penile hardness for sexual intercourse, can be eliminated with different treatment methods. Best method varies from case to case, so we wıll be determining the best option with you after your medical examination. Penile Prosthesis, also known as the happiness stick, is accepted as the last stage treatment method in erection problem.

After a detailed physical examination in patients who apply to a doctor with erectile dysfunction, treatment is planned as a result of laboratory examinations, advanced techniques such as penile Doppler ultrasound, angiography and penile cavernosometry.

The first choice in treatment is PDE5 inhibitors. In addition, some simple surgical treatments, medications and supportive practices are used. In cases of erectile dysfunction that do not respond to these treatments, a solution is offered to patients with penile prosthesis surgery.

Penile prosthesis, which is surgically placed into the penis, is a medical material.

Erectile dysfunction problem is eliminated by replacing the spongy tissue called corpus cavernosum in the penis with rigid or inflatable cylinders. This material is hidden inside the body and does not change the external appearance. From the outside, they are difficult to understand.

There are basically 2 types of penile prostheses;

Bendable (Malleable) Prostheses: These are prostheses that are permanently rigid and consist of a pair of parts. It is far from natural softening and natural hardness. They are preferred in elderly patients or when there are physical barriers that prevent the use of prosthesis. Andrologists may recommend implantation of a one-piece penile prosthesis if some patients have an increased risk of infection.

Inflatable Prostheses: Also called 3-piece penile prosthesis. This urological prosthesis, which can be defined as fluid-filled devices, was previously produced in 2 parts. However, due to the inadequacy of 2-piece prostheses, they are now produced as 3-pieces.

Three-Piece Penile Prosthesis: It consists of body cylinders placed in the penis, a pump placed between the ovaries and a fluid reservoir placed in the abdomen. There are types that can only expand transversely or both transversely and longitudinally. As a result of squeezing the pump several times during sexual intercourse, the liquid fills the cylinder from the reservoir in the abdomen and swelling occurs.

Thus, hardening is achieved. The return of the liquid back to the chamber takes place again through the pump. Three-piece urological prosthesis stands out as the most preferred penile implant, which gives the closest result to natural hardening.

In recent years, with the developing technology, prostheses that expand transversely and longitudinally have been produced. In this way, patients avoid shortening the length of the penis they experience due to erectile dysfunction.

Urine is controlled with a catheter placed in the bladder before the operation. Penile prosthesis implantation is performed by making a small incision between the penis and the ovaries. Appropriate sized cylinders, which are determined individually by a urology and andrology doctor who are experts in this field, are placed in the corpus cavernosum. Thus, in men, the cavities that provide hardness by filling with blood are filled with penile implants and the erection problem is eliminated.

The operation time is about 60 minutes. The hospital stay is 1 or 2 days, depending on the patient's condition.

The patients for whom the happiness stick will be made should have good blood sugar control and should not have urinary infection. For this reason, some tests are requested from the patients and their compliance is checked. In some patients, surgery may be delayed. Patients who are deemed suitable for surgery are checked by the anaesthesiologist one day before the surgery. A one or two night stay is required after the operation. If the patients have come from out of town or from another country, they can return to their place of residence two or three days after the operation. It should not be used for 6 weeks after the prosthesis is fitted, then it can be used safely. 

Most patients do not feel serious pain, still there may be mild pain that responds to painkillers.

Penile implant surgery carries the risks of every surgical operation. These can be listed as follows;

Risk of Infection: There is a possibility of infection in every patient, especially in patients with spinal cord injury and diabetes history.

Adhesion to the skin or erosion: In rare cases, penile implants may adhere to the internal tissue and damage the skin. Or it may separate from the skin and cause problems.

It is not a common situation in state-of-the-art penile prostheses. But there is always a risk of malfunction. For this problem that can be experienced in the form of fracture, removal, repair or replacement of the prosthesis is also possible with surgical intervention. Since the prostheses are guaranteed for life, there will be no additional cost to the patient if they need to be replaced again.
